Rifaximin is the preferred antibiotic for SIBO- although the success rate is about 40% for SIBO-D and even less of SIBO-C. For this reason, SIBO-C treatment involves a combination of two antibiotics- for example rifaximin and metronidazole (or neomycin). Guar Gum appears to increase the efficacy of rifaximin.

Even the most commonly used antibiotic for SIBO, Rifaximin (Xifaxan) has reported success rates of around 30% in methane-producing subjects, when used on its own. However, the combination of Rifaximin and Neomycin together has produced success rates of up to 87%.

The primary antibiotics used are Rifaximin (Xifaxan) and Neomycin. They are almost completely non-absorbable which means they stay in the intestines, having a local action and don't cause systemic side effects, such as urinary tract infections.

Of these antibiotics, puromycin, pseudomonic acid and neomycin appear to be most effective in treating methane SIBO and methane overgrowth 3. This may explain why your treatment for SIBO with methane overgrowth may not have been successful if you were prescribed a different and more traditional antibiotic protocol.

What would suggest as the best combination of herbal antibiotics for methane dominant SIBO? There are so many options and I am trying to find the right combination to get rid of my bloating, constipation, fatigue, acne (prescription meds for acne not working like it use to) and now shortness of breath because I am so bloated all the time.
